Use this four-page form to track student writing progress from the beginning of the year to the end. Collect and assess three samples of writing, at the beginning, middle, and end of the year, and write goals for the student after each assessment. For each assessment period, use a different color pen for filling in the date, scores, and comments to pinpoint key information and track progress at a glance—for instance, green for the beginning of the year, red for the middle, and blue for the end!
Teaching Tip:
The filled-out form, along with the samples of work and the assessment, can be placed in a folder and shared with parents. At the end of the year, pass the folder to the student’s new teacher. That way, at the first conference of the following year, the student’s new teacher can review the previous year’s folder with parents, and share the beginning-of-the-year assessment results, as noted on a new Year-Long Student Writing Record form.
